SMS Terms & Conditions

These SMS Terms & Conditions ("Terms") describe the text messaging program offered by Fifth & Heart Homecare (the "Program"). By opting in, you agree to these Terms and our Privacy Policy.

Program Description & Message Types

When you opt in, you may receive text messages related to:

  • Appointment reminders and confirmations

  • Intake and assessment coordination

  • Scheduling updates and caregiver arrival notifications

  • Service and safety alerts

  • Billing or account notifications

  • Customer support messages

  • Optional promotions or updates about services (marketing texts sent only with your express consent)

Messaging frequency may vary. Messages are sent via a 10-digit long code (10DLC) from our business number.

Message and data rates may apply. Check your mobile plan for details.

Opt In / Opt Out / Help

  • Opt In: You may opt in via our web forms, by texting a keyword we provide, or by providing written consent through other channels (e.g., in-person or over the phone where we capture consent).

  • Opt Out: Reply STOP to any message to opt out. You will receive a final confirmation message.

  • Help: Reply HELP for help, or contact us via www.fifthhearthomecare.com.

Consent is not a condition of purchase.

Eligibility & Your Responsibilities

  • You represent that you are the subscriber or customary user of the mobile number you provide and that you have authority to consent to receive texts at that number.

  • You agree to notify us if you change or deactivate your mobile number.

  • Device compatibility and wireless coverage are not guaranteed.

SMS consent is not shared with third parties or affiliates.

HIPAA Notice: SMS is not a secure or HIPAA-compliant channel. Do not include protected health information (PHI) in text messages. We will not request PHI via SMS.

Supported Carriers & Delivery

Delivery of messages is subject to effective transmission by your carrier and is not guaranteed. Carriers are not liable for delayed or undelivered messages.

Changes to the Program or Terms

We may modify or terminate the Program or these Terms at any time. Continued participation after changes constitutes acceptance of the revised Terms.
